This position offers an optional $10,000.00 award for a 3 year commitment to FHN.

Job Summary: Supervises the overall clinical and operational functions of FHN Family Counseling Center for the Freeport office(s) including: co-management of external billing/clinical/regulatory audits, provide direct and group supervision for targeted clinical, prevention, administrative and supervisory staff. Attend separate board meetings. Oversee compliance with internal and external regulatory guidelines. Maintain knowledge and facilitate implementation of national mental health legislation. Server as a bridge between hospital services/staff and mental health needs/processes.

Job Responsibilities:

  • Direct Monthly Meetings: Clinical staff- one hour/month, nursing staff - one hour/month, Staff training - organize annual training roster. Each training is one hour per month.
  • Facilitate Quarterly Meeting for all FCC Staff, Facilitate monthly Supervisor/Leadership meeting
  • Direct clinical services: Individual/marriage/family therapy, Forensic Evaluations for the Court, PAS Screens for the nursing homes
  • FCC Joint Commission Team Member: Chair the Infection Control, Patient Safety, Medication Management, Care Treatment and Services chapters. Insure policies and process are compliant with JC standards. Co-Facilitate Periodic Performance Reviews with VP Quality, Co-Manage external JC "unannounced" audit. Attend FCC JC monthly meetings.
  • Review and sign off on clinical documentation of unlicensed staff: Each mental health assessment, treatment plan, clinical notes, and psychology assessments
  • Policy creation and periodic review
  • Review/Write Grant Program Plans and Summary Reports: CFY, CCBYS, SASS, DCFS, PSR, DFI
  • Maintain 5 state Databases for Client information: DATSTAT, E-Cornerstone, SASS, SISS Online, and UHS
  • Manage Social Security Payee Client accounts: approve check requests, facilitate initiation or termination into the program
  • Complete Performance Evaluations, manage staff timecards via Laborworkx, hiring, disciplining, dismissal of staff, train new staff members. Assign and train staff mentors.
  • Provide Community and Hospital education/orchestrate community events: National Depression Screening Day, Operation Give Back, Career Day, Attend Hospital Department meetings, Facilitate Quarterly CONTACT training
  • Attend various FHN and external Committee Meetings (in person and telephonically): Joint Commission Core, Patient Safety, ad lib DTDC/EHR meetings, Continuity of Care, Physician Orientation, Medication Safety referrals, Leadership Forum
  • Program Development: Responsible for fiscal matters and oversight, needs analysis, staffing, etc.
  • Electronic Medical Record development and maintenance
  • Primary contact for NIHP mental health referral for approval
  • Community Involvement
  • Meet with Local Boards regarding Funding/Programs: Freeport 708 and FCC
  • CBHA regional meetings attendance: 8 annually, participate in IHA Rural BH subcommittee

Education/Experience: Master's of Social Work degree from an accredited school of social work, a master's degree in a human services field from an accredited school, or a master's degree in Business Administration or Health Administration. Prefer five to eight years prior field-related social work or human service administration experience.
