Minimum Qualifications: * *Education*: Bachelor's degree with progressive experience or a Master's degree in Physical Therapy. * *Licensure*: Must be licensed to practice as a Physical Therapist in Illinois. * *Experience*: Previous experience in home healthcare or familiarity with conditions of participation is preferred. * *Additional*: Must have a current driver’s license and reliable transportation to travel to and from patient sites.
