About the Position Job Title: Registered Nurse Profession: Nursing
Specialty: Home Health Duration: 13 weeks Shift: 8:00 AM - 5:30 PM
Hours per Shift: 8 Experience: Home Health experience required;
strong Med Surg experience with some Home Health experience
considered License: Active Illinois nursing license required
Certifications: BLS required, COVID and Flu vaccination required
Must-Have: Valid driver's license and car insurance OASIS and start
of care experience strongly preferred Description: Prescribes,
coordinates, and delivers care through the nursing process.
Maintains the standards of professional nursing. Supervises CNA and
coordinates care with other healthcare workers as assigned.
Provides comprehensive initial assessments to all clients.
Continually assesses physical, psychosocial, environmental, and
community resources available. Utilizes the principles of growth
and development over the life span. Initiates Plan of Care,
re-evaluates the need for care, and modifies Plan of Care as
ordered or needed. Establishes discharge plans with patients,
physicians, other disciplines, and caregivers. Completes OASIS data
as required. Provides patient care using current federal and state
regulations. Assesses educational needs of patients, families, and
caregivers. Implements appropriate teaching strategies based on
assessments. Completes documentation according to regulatory
guidelines. Promotes a clean and safe environment of care,
utilizing error prevention habits. Provides the highest standard of
privacy and confidentiality in matters involving patients,
coworkers, and the organization. Requirements Required for
