As a Home Health Physical Therapist, you'll work in a culture where management is supportive, and you are empowered to advocate for your patients. /n /nHome Health Physical Therapist Overview: /n/n /n/t - Competitive Compensation: Earn between $75 and $150 per visit ( mileage) /n/t - 1:1 therapist-to-patient ratios where you focus on delivering exceptional, personalized care /n/t - Flexible scheduling, variety of assignments, and work-life balance /n/t - Access to online training, CEU opportunities, and tuition discounts through Rasmussen University /n/t - Baylor Program opportunities also available (Every other Saturday and Sunday, per visit ($200 SOC, ROC $185.00, etc.) /n /n/n Your Role and Responsibilities as a Home Health Physical Therapist: /n/n /n/t - Provide physical therapy to patients who have lost mobility, strength or functionality in facilities such as nursing homes, rehab centers, assisted living facilities, clinics and hospitals /n/t - Work with a team of physicians, nurses, CNAs, OTs and SLPs to provide excellent patient care /n/t - Assist physician in evaluation of patient function and performance, as well as equipment needs /n/t - Conduct assessments, document progress and determine if therapy services should be modified /n/t - Assist patient with exercises and hands-on therapy to manage pain and increase mobility /n/t - Educate patients and family on established goals, exercises and self-care to ensure progress /n /n/n Qualifications for an Interim HealthCare® Physical Therapist: /n/n /n/t - Graduate of an accredited Physical Therapist Program with an active PT license in IL /n/t - Minimum 2 years of physical therapy experience, ideally in home healthcare /n/t - Current CPR and BLS certifications /n/t - Knowledge of state and federal home health regulations /n/t - Strong clinical judgment, excellent communication skills, and a compassionate approach /n /n/n /nWhy Join Interim? /nAs the nation's first home care company, founded in 1966, Interim HealthCare® is seeking dedicated Home Health Physical Therapists (PTs) who are passionate about providing high-quality, personalized care.
