Friday Position Summary We're seeking a skilled Contract Physical
Therapist (PT) to join a Skilled Nursing Facility (SNF) in Pleasant
Prairie, Wisconsin. In this role, you'll evaluate patients, develop
individualized therapy plans, and help improve mobility, strength,
and functional independence. Key Responsibilities Evaluate patients
and develop individualized physical therapy treatment plans Provide
therapeutic exercises, gait training, and mobility interventions
Monitor patient progress and adjust treatment plans as needed
Document evaluations, treatments, and progress notes accurately
Collaborate with interdisciplinary care teams to optimize patient
outcomes Educate patients and caregivers on exercises and mobility
strategies Ensure compliance with facility policies and state
regulations Required Qualifications & Skills Active Wisconsin
Physical Therapist license (required) Degree in Physical Therapy
from an accredited program Experience in a Skilled Nursing Facility
(SNF) preferred Strong clinical and documentation skills Ability to
work independently and as part of a team Effective communication
and patient care skills Core Competencies Patient-centered
rehabilitation Clinical assessment and treatment planning Time
