Job Licensed Therapist Key Rsponsibilities: Perform on-site and
telehealth clinical counseling services to patients of Urban
Wellness, as scheduled, and in accordance with company policies.
Conduct and regulate counseling services in a professional manner
so as to maintain and increase the good will and reputation of
Urban Wellness. Abide by all laws, rules, regulations, and codes of
ethics that are binding upon or applicable to the services
performed for Urban Wellness. Respond to patients’ requests for
service or calls within established timeframes whether such
requests are made in person, by phone, voicemail, etc. or through
the Urban Wellness website. Respond to such requests by meeting,
evaluating, and providing services to such patients. Complete
required documentation in an efficient and timely manner. Complete
written records for each patient including, but not limited to:
intake notes, progress notes, treatment plans, termination notes,
contract notes, and other forms or documents which may be needed or
required from time-to-time by Urban Wellness or third parties in
conjunction with the treatment of the patient within a timely
fashion (by Friday of each week). Maintain accurate, up to date
patient files, records, plans, and reports. Communicate and
collaborate with a patient’s treatment team (i.e., PCP,
psychiatrist, school, etc.) as necessary. Charge and collect
payments from patients for services provided consistent with the
policy and rate for such services as established by Urban Wellness
Skills that May Make You a Great Fit for this Job - Strong working
knowledge of human behavior and performance; individual differences
in ability, personality, and interests; learning and motivation;
psychological research methods; and the assessment and treatment of
behavioral and affective disorders. Strong working knowledge of
principles, methods, and procedures for diagnosis, treatment, and
rehabilitation of physical and mental dysfunctions, and for career
counseling and guidance. Strong working knowledge of group behavior
and dynamics, societal trends and influences, human migrations,
ethnicity, and cultures and their history and origins. Team player.
Collaborate well with team members in support of our clients and
community. Demonstrate a passion for working with others (being
collaborative) and helping fellow team members grow and learn.
Ability to accept and act on candid performance feedback. Ability
to remain calm in stressful situations and during crises.

systems. Scheduling Expectations: Full-time: 30-hours per week; 25
clinical hours Evening/Weekend Hours: Either 2 evenings 4 pm to 8
pm or 1 evening and 1 weekend Hybrid work schedule: All clinicians
are required to work no less than 2 days in office Our Office
Locations: As of June 10, 2025: We are hiring in the following
three Chicago neighborhoods; Edison Park, Sauganash, Old Irving
Park. We have an office in Forest Park and are not currently adding
to this team. Candidates are asked to share which location they
would prefer. Minimum Required Education & Experience: Master’s
Degree or Doctoral Degree in counseling or social work; and Active,
unrestricted license (PhD, PsyD, LCSW, LMFT or LCPC) in the State
of Illinois Must be fully licensed. A copy of your license must be
provided to Urban Wellness before working with patients. Three (3)
years’ experience as an independently licensed clinician;
fully-unrestricted licensure. Have at least one niche market for
counseling services (i.e., children, teens, trauma, anxiety, grief,
anger management, etc.). Current approval as an in-network provider
with BCBS is preferred What it's like working at Urban Wellness:
Compensation, Benefits & Perks: Compensation Fee-for-session pay;
flat rate per session based on years of experience and licensure.
Fee-for-session rates range between $41 and $59 per session

Work Location: Primarily in-person, in office at an assigned Urban
Wellness location Telehealth may be provided to meet the needs of
specific clients or circumstances Scheduling Requirements: Be
available for client sessions work (2) evenings per week or one
evening and one weekend (6 hours) per week. Note: Evenings defined
