Position TravelNurseSource is working with Cynet Health to find a
qualified Diabetes RN in Rockford, Illinois, 61103! Job Title:
Certified Diabetic Educator Profession: Nursing Specialty: Diabetic
Education Duration: 13 weeks Shift: Monday to Friday Hours per
Shift: 40 Experience: Minimum 2 years License: Licensed as an RN in
IL and WI Certifications: Approved CPR certification required
within 90 days; CDCES certification required Description: Graduate
of an accredited nursing program. BSN preferred. Provides
compassionate, culturally sensitive, holistic nursing care to
patients in a clinical setting. Supports the nursing mission,
practice values, and strategic plan. May include variable shifts,
weekends, holiday, and/or on-call hours. Performs other duties as
assigned, including floating to various units based on identified
needs and clinical competency. In collaboration with the
multidisciplinary diabetes management team, provides diabetes
education to patients and families living with diabetes mellitus.
Utilizes the nursing process and collaborates with
interdisciplinary team members in caring for patients. Administers
prescribed medications and treatments in accordance with approved
nursing techniques, policies and procedures, standards of care, and
state laws. Documents according to standards of care. Develops,
initiates, and executes a patient plan of care according to the
individualized needs of the patient, as prescribed by the physician
and/or clinic policy, including patient and family instruction.
Participates in department or unit performance improvement
activities, which may include shared governance. Utilizes clinical
expertise and guidance to other units or departments in support of
the patient's longitudinal plan of care. Utilizes critical thinking
regarding patient safety and patient experience and appropriately
escalates to the appropriate chain of command. Acts as a resource
to staff on patient education program/materials development and on
patient education issues or problems. The Educator is a patient
advocate who addresses clinical, psychosocial, and financial
