Job Description:
The Registered Nurse provides direct and indirect home care,
following policies and physician plans to promote patient
independence and health.
Responsibilities include administering medications, documenting
care, conducting assessments, and educating clients on self-care
and safety.
Supervises caregivers, ensures compliance with regulatory
standards, and manages medication protocols and documentation.
Performs community-based nursing support, collaborates with
healthcare teams, and participates in staff training.
Qualifications include a nursing degree, licensure, CPR
certification, and preferred home health experience.
Strong communication, organizational skills, and the ability to
handle confidential information are essential.
Additional duties may involve after-hours coverage, case
management, and ensuring quality care tailored to individual
needs.
