We are seeking a dedicated Licensed Practical Nurse (LPN) with the
highest standards of excellence, who seeks the opportunity to work

LPNs provides nursing care to the terminally ill hospice patient
following plan of care (POC) for assigned patients, as needed. You
will provide assistance and understanding to the family in the home
care situation and in time of bereavement. In addition you will
provide nursing care, education, and support to the patient/family
and/or caregivers.
- Provides professional, courteous, nursing services to the
patient/family.
- Supervises the administration of medications by family or other
responsible caregivers; teaches the proper techniques of
administration and record keeping.
- Observes signs and symptoms and reports to the physician and
interdisciplinary group (IDG) members any unexpected changes in the
patient's physical or emotional condition. Makes home visits to
provide direct care as needed. Provides patient visits at the
request of the patient and/or family or facility nurse during on
call hours.
- Maintains a high standard of quality of care according to
hospice training, guidelines, and expectations.
- Demonstrates tolerance to various and differing individual
lifestyles without imposing his/her own values on the
patient/family unit.
- Reports to supervisor for daily schedule, reporting pertinent
changes to the supervisor, or the Primary Nurse during the shift
and at the end of his/her shift.
- Maintains an up-to-date knowledge and understanding of the
local, state, and federal laws impacting upon the practice of
nursing in the hospice setting.
- Adheres to professional standards of practice for nursing.
- Assists the Hospice RN Case Manager in the implementation and
coordination of the Care Plan. Assesses the patient's physical and
emotional condition and provides patient care as directed.
- Teaches the patient and family as required to maintain safe
environment.
- Maintains current knowledge of Medicare/Medicaid benefits,
private insurance, supportive care, continuous care and in-patient
requirements.
- Performs specific tasks as follows but not limited to, personal
hygiene (bath, shower, hair, nails, etc.), passive range of motion
and positioning, enemas (prepared, tap water, soap suds),
dressings, wound irrigations and decubitus care, colostomy care,
administration of medications and instructing patient and family in
medications administration, tracheostomy care and suctioning.
- This position will be supervised monthly by an RN.
- Participates in nursing on-call rotation as assigned.
- Practices within the scope of Licensed Practical Nurse license.

base wage range for this role is $28.00 - $37.00. However, final
base pay will be carefully determined based on several factors,
including education, experience, knowledge, skills, and abilities,
as well as internal equity and alignment with market data.
Additionally, this role may be eligible for other forms of
compensation and benefits.

Requirements:Education: Completion of a formal Practical Nurse
Program.Training and Licensure: Current Practical Nurse Program
(LPN) license issued by the Illinois Board of Nursing
required.Knowledge/Skills/Ability: Ability to work independently,
make accurate, and at times, quick judgements. Ability to respond
appropriately to crisis in a community setting. Acceptance of and
adaptability to different social, racial, cultural, and religious
modes.Experience: A minimum of two (2) years of nursing experience
which includes basic nursing skills with the administration of
medications. Home care experience preferred.Other: Local residence
with ability to respond to on-call within 30 minutes. Meets agency
requirements for criminal background check.
