ProMedica Hospice formerly Heartland Hospice has an exciting
opportunity for a RN to join our established hospice agency serving
the Rockford and surrounding areas. This full-time position
requires working Monday through Friday 12pm-830pm or 2pm-10:30pm.
In this role you will providing care to our patients in their homes
as needed. Limited on call rotation is required.
TheRN - Registered Visit Nurse(hospice) position is a nurse who
administers skilled nursing care to clients requiring professional
nursing services in their homes and teaches/supervises the family
and other members of the nursing team. These services are performed
in accordance with the physicians orders and under the direction
and supervision of the Clinical
Manager and the Case Manager.

Educational Requirements
Currently licensed as a Registered Nurse in the state and in good
standing with the Board in which he or she will practice.
