PURPOSE OF THE POSITION: Work to obtain signed consents from the
parent so child may receive Medical, Vision and / or Dental care.
Will assist nursing staff to obtain immunization and physical exam
information from parents. This individual will make phone and
community contacts to set up medical, vision and dental services
for students. This individual will accompany students, who are
transported in District authorized vehicles, to receive medical,
vision and dental care. The applicant must be able to work
independently to develop and implement an effective and efficient
medical, vision and oral health program. This individual must
possess knowledge of HIPAA, FERPA, IL School code, IDPH rules and
District 205 regulations as they pertain to student's right to
confidentiality, health, and safety. The position will require
working with community partners to develop School Based Health
Centers throughout the District. This individual may be asked to
assist with other duties, as directed, by the immediate
supervisor
SUPERVISORY RESPONSIBILITIES: None
DUTIES AND RESPONSIBILITIES: To perform this job successfully, an

* Ensures that all actions are in concert with the Board policies
as well as the mission statement, beliefs, objectives and
parameters found in the Board's strategic plan.
* Participates in job training and professional growth
opportunities in order to enhance ability to perform the essential
functions of the job.
* Performs other related duties as assigned for the purpose of
ensuring an efficient and effective work environment.
* Parent Liaison duties
* Contact parent to explain results of medical care, vision
screening, and dental screening, identified need for medical care,
glasses, or restorative dental care.
* Explain to parent the details of the medical care, vision
follow-up, or dental care proposed for their child
* Ensure parent understands services that are to be provided to
their child
* Obtain consent for these health services.
* Provide follow-up contact to parent regarding medical treatment,
vision exams, or dental care rendered to their child.
* Collaborate with District Communications Department to help keep
parents abreast of changes in health requirements for school
compliance.
* Community Liaison duties
* Coordinate dental and health services with community partners
* Accompany from serving school to appointment and return to
serving school. Supervise students in District authorized vehicle
for their dental and medical appointments
* Encourage positive student behavior while in the community
* Keep accurate data and report on a monthly basis to immediate
supervisor - numbers of children served by this service on a
monthly basis/type of service/completion of treatment
* Develop professional relationships with community partners
* Assists with implementation of proposed School Based Health
Center.
* Collaborates with community partners to write grant proposal for
development of multiple School Based Health Centers throughout the
District.
* Health Office duties
* Assist school nurses in bringing students into compliance with
physical exams and immunizations
* Assist school nurses in bringing students into compliance with
dental and vision exams as required by the state of Illinois
* Provide school nurses with miscellaneous Health office support as
needed and directed by the Health Services Supervisor
* Administrative Duties
* Manage, and support School Based Health Center
* Continually assess efficiency of program and be cognizant of
financial constraints.
* Implement, and analyze data storage and retrieval procedure on a
regular basis.
* Develop and provide presentation for Rockford Health Council
Board of Directors annually
* Create public relations articles and materials to be used to
market health programs at District events.
* Coordinate oral health program awareness campaign throughout the
District.
* Assist with Grant writing to extend health services to
students.
* Coordinate and oversee School Based Health Center Advisory
Board
* Provides supplemental/discretionary reports, data and
materials.
QUALIFICATIONS:
Minimum High School Diploma, Bachelor's Degree preferred.
KNOWLEDGE, SKILLS AND ABILITIES:
Must be able to work independently in order to accomplish the
intended goals of the Health Service program. Must make connections
within each school building and within the community to move the
health program forward. Must possess excellent communication
skills, both face-to-face and using written language. Must be able
to develop data collection and tracking systems which are used to
report progress.
WORKING CONDITIONS:
Moderate in-district travel as well as intermittent in-state and
out-of-state travel. Intermittent prolonged and irregular hours of
work.
PERFORMANCE STANDARDS: Growth of the health program is evidenced by
a reduction in the number of students out of compliance by the
District's established exclusion date, a reduction of the number of
students out of compliance with the state mandated vision exam
requirements, and an increased numbers of students receiving dental
screenings and/or restorative care. Performance is measured by
increased numbers of students receiving medical care through the
School Based Health Center and by the number of community partners
participating positively in health promotion programs.
