Promotes the physical and mental health of residents by applying
the nursing processes. This is done as a member of the
interdisciplinary treatment team and has the goal of assisting
residents to maintain an optimum level of health. The nurse
coordinates and monitors medical services in accord with the
requirements of regulatory agencies and in response to individual
resident need. We are looking for a compassionate and reliable
nurse. The right candidate will be responsible for leading a strong
team of CNAs in a fair and professional manner. Must have a good
attendance record, good attitude, and a valid/current Illinois
license. Job Type: Part TimePOSITION RELATIONSHIP: Reports to
Director of Nursing or Designee. EXPOSURE CATEGORY: Category 1:
Routine tasks involve exposure to blood and body fluids/tissues
which may contain blood. Use of Universal Precautions/Standard
Precautions is required. ESSENTIAL DUTIES AND RESPONSIBILITIES:
Include the following. Other duties may be assigned. Management:
Manages own time to accomplish responsibilities. Has responsibility
for all nursing activities of his/her unit and coordinates care
between shifts. Uses critical thinking and problem-solving
techniques when managing responsibilities. Presents as a role
model. Displays a positive attitude and is supportive of peers and
administration. Assists the Director of Nursing in managing
performance of nursing personnel. Adjusts work assignments and
calls in off-duty personnel to maintain adequate staff on all units
during the shift as necessary. Assists and monitors new employee
training process and provides feedback to the Director of Nursing.
Supports and participates in the facility Quality Improvements
program, including monitoring QA criteria. Assures facility
practices on their shift are consistent with regulatory
requirements. participates in committees as assigned. communicates
to the Director of Nursing or designee all resident and/or staff
issues which may directly impact the nursing department or
facility. attendance is an essential function of this position.
perform other duties as assigned by the Director of Nursing or the
Administrator. Care Delivery: participates in the care planning
process. communicates residents' need to the Director of
Nursing/designee. assures resident rights are respected at all
times. assures implementation of residents' individualized care
plans. implement and direct care as appropriate or necessary.
Ensure that nursing assessments, including basic assessment, skin
assessment, fall risk assessment are complete within 24 hours of
admission and as necessary. Completes pain assessment as indicated.
Institute, review, and update nursing care plans, involving patient
and family needs. Care plans must be ready prior to scheduled care
plan conference. Assign duties to ancillary staff based on resident
needs, available staff and unit needs. Maintain and review nursing
observation records, including quality and time interval, according
to facility policy. Evaluate patient conditions and notify
supervising nurse of changes. Evaluate quality of nursing care.
Make resident rounds with physicians and with supervisor on duty.
Required to wear personal protective equipment which includes, but
is not limited to, gloves, gowns, goggles, and/or masks. Attend the
following required inservices annually; Life safety, proper body
mechanics, infection control, T.B., material safety data sheets,
and residents rights. Attend inservices as assigned by immediate
supervisor. Recognize fire and safety hazards; observe and obey
fire and safety precautions. Interpret and support facility policy
to staff, residents and visitors and insures compliance. Observe
and report to the supervisor any repairs needed in the physical
plant or unit. Fill out work orders as needed. Utilize/order proper
equipment and supplies. Make compliance rounds. Review chart audits
and make necessary changes and/or corrections. Complete necessary
documents, reports, records as indicated. Follow the department
dress code. Participates in the admission of new residents.
Clarifies initial or hospital discharge medication orders and
contacts the Physician to review the resident's status, treatment
orders and medication plans. Develops plans to ensure that the
medical and health care needs of residents are met. This includes:
Ensuring appropriate medical follow-up Providing special
instructions to resident and facility staff related to medical
diagnosis and plan of care as well as allergies, medication, diet,
special emergency plans related to ongoing medical conditions.
Coordinating and reviewing lab work. Coordinating care with
clinics, outside physicians and hospitals. Health teaching.
Functions as part of an interdisciplinary team attending medication
rounds and team meetings for Interdisciplinary Team (IDT) and
Quarterly Review. Is available for ongoing consultation to the team
regarding medical problems and health care. Documents services in
accordance with facility policy and requirements. Promotes resident
empowerment and self-care activities. ADDITIONAL DUTIES AND
RESPONSIBILITIES Covers for peers during vacations and at other
times of facility need. Participates in facility training
activities. Participates in the orientation of new nursing staff.
Attends staff, discipline and committee meetings as assigned by
Supervision. Reviews and signs incident reports and provides
follow-up. Assumes responsibility for professional development. All
other duties as assigned by supervision. SUPERVISORY
RESPONSIBILITIES: Responsible for supervising assigned unit and/or
facility. QUALIFICATION REQUIREMENTS: To perform this job
successfully, an individual must be able to perform each essential

EXPERIENCE: Minimum Associates Degree in Nursing. Two years of
experience working as a registered nurse and awareness of the needs
of the geriatric resident. preferred.
