providers and local businesses POSITION SUMMARY: As the Midwests
premier LGBTQ health and human service organization, Howard Brown
Health exists to eliminate the disparities in health care
experienced by lesbian, gay, bisexual, and transgender people
through research, education, and the provision of services that
promote health and wellness. The Diabetes Care Navigator supports
the health and wellness of patients living with diabetes accessing
care at Howard Brown Health by providing on-call patient support
aimed at assisting patients with navigating access to medication,
access to diabetes care testing and treatment supplies, and
connecting patients to long-term supportive services when
appropriate. The patient navigator supports patients by providing
patient-centered health education, advocacy, resource navigation,
and referrals to other health and wellness services. PRINCIPAL
DUTIES AND RESPONSIBILITIES Complete brief, immediate needs
assessments with Howard Brown Health patients who may benefit from
assistance in accessing medications, testing and treatment
supplies, and other relevant medical and social services related to
diabetes and related medical conditions provided by Howard Brown
Health, community organizations, medical supply vendors, and
insurance companies. Provide information, education, and emotional
support to patients in a compassionate, culturally sensitive, and
appropriate manner. Evaluate patients abilities to understand
health plans and provide relevant health plan navigation education
to patients to improve health outcomes; consult with medical
providers regarding patient needs. Consult and collaborate with a
multi-disciplinary team including primary care providers,
behavioral health staff, nursing, MAs, and other Howard Brown staff
involved in client/patient care, in addition to those across
different institutions and agencies. Coordinate access to
medication, diabetes testing and treatment supplies, and related
durable medical equipment with patient, medical providers, medical
supply companies, and insurance companies. Assess financial need of
patients and facilitate patient treatment financial support; help
with applications,especially as it relates to prescription
assistance programs. Provide education, information, and referrals
(health insurance, prescription assistance programs, etc.) to
patients on an ongoing basis; locate, identify, and utilize new
services and resources. Develop and maintain required records,
reports, and statistical data. This includes completing activity
tracking reports, progress notes, resource receipts, and tracking
health maintenance of each client. Maintain accurate understanding
of the most current scientific knowledge regarding the treatment
and prevention of diabetes by participating in ongoing continuing
education, attending trainings, in-services, professional
development events, and by self-education. Attend and participate
in all group consultation/case conferences, social service
department meetings and trainings, team meetings, individual
supervision, and any other required staff meetings and trainings.
Will follow role assignments that are in alignment with the Patient
Centered Medical Home Standards, and participate in all Patient
Centered Medical Home efforts. Maintain the highest level of client
confidentiality. Other duties as assigned. Qualifications
QUALIFICATIONS, KNOWLEDGE, SKILL REQUIRED: Required: High school
diploma or GED with 3-4 years experience or Bachelors degree with
1-2 years experience. Experience working in a community-based
medical/social services setting Experience with diverse communities
Desire to work for and within Lesbian, Gay, Bisexual, Transgender,
and Queer communities. Preferred: Prior case management experience
Prior knowledge and experience working with clients living with
diabetes and other chronic diseases Bilingual-English/Spanish
language proficiency Working Conditions: May require travel between
clinic sites May require evening and Saturday shifts to align with
clinic hours Requires ability to speak audibly and listen actively
Requires ability to use computers, telephones, and other office
equipment Requires ability to sit for extended periods of timeby
